Guidelines used to recommend that parents avoid feeding their infants common allergenic foods in their first year, but expert advice has recently changed. Emerging evidence from randomized controlled trials suggests that early introduction of allergenic foods, particularly peanut, is protective against the development of food allergy. In their clinical practice guideline, Dr. Liu and colleagues provide recommendations on the diagnosis and management of deep vein thrombosis (DVT), including anticoagulation, thrombus removal strategies and inferior vena cava filters. The poor outcomes seen in patients with iliofemoral DVT treated with standard anticoagulant therapy have led to exploration of other treatment options.   • Identifying skull fracture in young children: a clinical decision rule

In a research article published in CMAJ, Dr. Gravel and colleagues derived and validated a clinical decision rule to identify skull fracture following minor head trauma in young children. The rule should make it possible to identify about 90% of skull fractures in young children with mild head trauma and reduce the use of radiologic investigations by about 60% compared with current practice.   • Family physician or obstetrician for delivery: no difference for moms and babies

In their observational study, Dr. Aubrey-Bassler and colleagues analyzed administrative data for all hospital births in Canada (except Quebec) between April 2006 and March 2009, comprising nearly 800 000 babies and 800 000 mothers. Instrumental variable analysis was used to adjust for unmeasured potential confounders. Whether babies were delivered by a family physician or an obstetrician made no difference to perinatal mortality or to maternal mortality and morbidity.
